An application continues in the application stage until an action is taken. This action consists of:

1. (1) Approval of application.
2. (2) Denial or rejection of application due to death, withdrawal of application, loss of contact, failure to provide or obtain information necessary to determine eligibility or ineligibility based on program regulations.
3. (3) Pending of Application.
