1. (1) The applicant to each program for the training of medical laboratory personnel shall submit the following information:

   1. (a) Personal history data:

      1. 1. Name, permanent address, local address, telephone number, parent’s or guardian’s name (address and telephone number if appropriate);
      2. 2. Date of birth, place of birth, citizenship status, and marital status;
      3. 3. Employment record;
      4. 4. Military service record (form DD214); and
      5. 5. Social Security Number;
   2. (b) A transcript of college credits where indicated; and
   3. (c) Evidence of good health which shall consist of a record of medical history and a complete physical examination certified by a licensed physician or nurse practitioner.
2. (2) All records of students shall be kept on file for a minimum of ten (10) years.
