1. (1) A person who holds a current license and does not intend to practice as an occupational therapist or occupational therapy assistant may apply to convert an active license to inactive or retired status.

2. (2) A person who holds an active license may apply for retired status in the following manner:

   1. (a) Obtain from, complete, and submit to the Board’s administrative office an affidavit of retirement form; and
   2. (b) Submit any documentation which may be required by the form to the Board’s administrative office.

3. (3) A licensee whose license has been retired may reenter active status by doing the following:

   1. (a) Submit a written request for licensure reactivation to the Board’s administrative office; and
   2. (b) Pay the licensure renewal fees and state regulatory fee as provided in Rule 1150-02- .06. If retirement reactivation is requested prior to the expiration of one (1) year from the date of retirement, the Board will require payment of the late renewal fee and all past due licensure renewal fees; and
   3. (c) Complete the continued competence requirements, as provided in Rule 1150-02-.12.
4. (4) Licensure reactivation applications shall be treated as licensure applications and review and decisions shall be governed by Rule 1150-02-.07.
