1. (1) An Individual Program Plan (IPP) must be developed and documented for each service recipient within thirty (30) days of admission or by the end of the third face-to-face treatment contact with qualified alcohol and drug abuse personnel, whichever occurs first, and must include:

   1. (a) The service recipient’s name;
   2. (b) The date of the IPP’s development;
   3. (c) Standardized diagnostic formulation(s) including, but not limited to, the current Diagnostic and Statistical Manual (DSM) and/or the International Statistical Classification of Diseases and Related Health Problems (ICD), and ASAM PPC;
   4. (d) Specified service recipient problems which are to be addressed within the particular service/program component;
   5. (e) Service recipient goals which are related to specified problems and which are to be addressed within the particular service/program component;
   6. (f) Interventions addressing goals; DRUG NON-RESIDENTAL REHABILITATION TREATMENT FACILITIES
   7. (g) Planned frequency of contact;
   8. (h) Signatures of appropriate staff; and
   9. (i) Documentation of the service recipient’s participation in the treatment planning process.
