- (1) The Facility shall have a procedure which shall ensure that no service recipient is enrolled in more than one (1) Facility at a time.
- (2) The procedure shall take into account requirements for service recipient confidentiality.
- (3) The Facility shall obtain a written release of information from the service recipient in order to check the records of every Facility within Tennessee and those Facilities within seventy-five
- (75) miles of the Facility, or those Facilities that participate with the Central Registry. The release of information shall state that its purpose is to obtain information and records developed during prior admission(s). Results of this check shall be maintained in the service recipient’s medical record. This check shall be duplicated if the service recipient is discharged and readmitted at any time.
