1. (1) The governing body of the Facility shall ensure the OBOT is administered and operated in accordance with written policies and procedures in the below listed subject areas and in accordance with these rules. Each Facility shall clearly identify the governing body, as defined in Rule 0940-05-01-.01(18) and Rule 0940-05-35-.02(2)(l), in its policies and procedures manual including the name and contact information of the governing body.

   12. (l) Reporting Requirements (0940-05-35-.17);
   13. (m) Patient Rights (0940-05-35-.18);
   14. (n) Community Relations (0940-05-35-.19); and
   15. (o) Personnel and Staffing Requirements (0940-05-35-.20).
