The facility must ensure that the following assessments are completed prior to the development of the Individual Program Plan:
(1) Assessment of current functioning according to presenting problem including a history of the presenting problem in the following areas:
- (a) Community living skills;
- (b) Living skills appropriate to age;
- (c) Emotional, psychological health; and
- (d) Educational level (including educational history).
- (2) Basic medical history and information.
- (3) A six (6) month history of prescribed medications, frequently used over-the-counter medications, and alcohol or other drugs.
- (4) History of prior mental health and alcohol and drug treatment episodes.
- (5) Assessment of whether client meets eligibility requirements for special education services in accordance with the State Board of Education Rules, Regulations, and Minimum Standards.
