1. (1) The Commissioner may revoke designation at any time with a written notice:

   1. (a) for good cause,
   2. (b) failure to meet minimum requirements,
   3. (c) violation of the law.
2. (2) A designated treatment resource may terminate the designation with a thirty (30) day written notice to the Commissioner.
