The following fees are nonrefundable and apply to all applicants and licensees.

2. (2) Biennial Renewal Fee $275.00
3. (3) State Regulatory Fee - To be paid upon application $ 5.00 and annually thereafter to be collected at biennial renewal from all license holders. ($10.00 biennially)
4. (4) Late Renewal Fee $ 50.00
5. (5) Replacement License Fee $ 25.00
