1. (1) Contract — any insurance policy, plan, or binder, including any rider or endorsement thereto offered by an insurer.
2. (2) Insurer — any insurance company, association, reciprocal or inter-insurance exchange, hospital or medical service corporation or plan, health maintenance organization, fraternal benefit society or beneficial association.
