1. (1) Grants shall be made on the basis of applications submitted to the Executive Director by the applicant. The applica- tion shall include at least:

   1. (a) List of proposed activities;
   2. (b) Amount requested;
   3. (c) Amount and source of matching funds;
   4. (d) Targeting elements (physical or economic);
   5. (e) Method of notifying the public of the program;
   6. (f) Criteria for choosing applicants;
   7. (g) Procedure for selection of contractor, if applicable;
   8. (h) Number of households to be assisted;
   9. (i) Itemized administrative budget.
