1. (1) Change of Name. The school’s program director shall notify the Registry’s administrative office in writing within thirty (30) days of a name change. The notice shall provide both the old and new names and must reference the license number, along with a certified or notarized photocopy of the document authorizing such a name change.
2. (2) Change of Address. The school’s program director shall notify the Registry’s administrative office in writing within thirty (30) days of a change of address. The notice shall provide both the old and new addresses and must reference the school’s name, telephone number, and license number.
