1. (1) The governing body must ensure that each record of the person supported contains the following:

   1. (a) A recent photograph and identifying information of the person supported;
   2. (b) The social security number of the person supported;
   3. (c) The legal competency status of the person supported, including the name of his/her legal representative (conservator, parent, guardian, or legal custodian), if applicable;
   4. (d) The sources of financial support of the person supported, including social security, veterans benefits, and insurance;
   5. (e) The sources of coverage for medical care of the person supported;
   6. (f) The name, address, and telephone number of the physician or healthcare agency providing medical services for the person supported;
   7. (g) Report of medical problems, accidents, seizures, and illnesses of the person supported, and treatments for such medical problems, accidents, seizures, and illnesses as they occur;
   8. (h) Report of significant behavior incidents of the person supported, and of actions taken;
   9. (i) Report of restrictive behavior-management techniques used on the person supported; and DEVELOPMENTAL DISABILITIES PERSONAL SUPPORT SERVICES AGENCIES
   10. (j) Written accounts of all monies received and disbursed on behalf of the person supported.
