1. (1) Assessments must be completed by using empirically reliable and valid instruments of diagnosis and evaluation.

2. (2) Diagnosis and evaluation services must be capable of assessment in the following areas:

   1. (a) Intellectual development;
   2. (b) Sensorimotor development;
   3. (c) Affective development;
   4. (d) Social development;
   5. (e) Communicative development; and
   6. (f) Physical development.

3. (3) Assessment of physical development must be completed by the following:

   1. (a) Physical examination;
   2. (b) Review of Medication history; DEVELOPMENTAL DISABILITIES DIAGNOSIS AND EVALUATION FACILITIES
   3. (c) Visual screening;
   4. (d) Audiological screening;
   5. (e) Nutritional screening; and
   6. (f) If determined necessary, a genetic evaluation.
