Satisfactory proof of experience shall consist of either of the following methods.

1. (1) A list of ten (10) wells the applicant has constructed or worked on during a minimum of the last two years prior to the date of making the application for a license. The information shall include for each well the following:

   1. (a) Name and address of the owner or owners of each well;
   2. (b) Location and intended use of each well;
   3. (c) Major construction features such as depth, type of casing, backfill, yield and water quality;
   4. (d) Date of completion; and
   5. (e) Work done by applicant and approximate customer cost.
2. (2) Copies of occupational licenses or certificates covering two years and indicating that the applicant has been engaged in the occupation for which a license is being sought.
