Requests for amendment of a Certified Fusion Machine Registration and/or License shall be filed in accordance with paragraph (2) of Rule 0400-20-14-.06 and shall specify the manner in which the registrant desires the Certified Fusion Machine Registration and/or License to be amended and the grounds for such amendment.
