The licensee or registrant shall submit a written report to the Division of Radiological Health, at the address given in Rule 0400-20-04-.07, within 30 days following any planned special exposure. The report shall inform the Division that a planned special exposure occurred and provide the information required by Rule 0400-20-05-.134.
