Fees include the following:

1. (1) Certificate of licensee, a fee not to exceed fifteen dollars;
2. (2) For each additional office or place of business, a biennial fee not to exceed thirty dollars;
3. (3) For each change of office or place of business, a fee not to exceed fifteen dollars;
4. (4) For each statement of registration of change of association, a fee not to exceed fifteen dollars; and
5. (5) For each duplicate license, if the original license is lost or destroyed and affidavit made thereof, a fee not to exceed fifteen dollars. Fees shall be set by the commission by rules promulgated pursuant to chapter 1-26.
