The agency shall maintain a file of:

1. (1) All license applications, issuances, denials, amendments, transfers, renewals, modifications, suspensions, revocations, and any administrative or judicial action pertaining thereto;
2. (2) Registrants possessing sources of ionizing radiation requiring registration under the provisions of this chapter and any administrative or judicial action pertaining thereto;
3. (3) All rules and regulations relating to regulation of sources of ionizing radiation, pending, or promulgated, and proceedings thereon; and
4. (4) All monitoring data gathered by or submitted to the agency.
