A designation that the licensee is a veteran must be indicated on a commercial driver license or a commercial learner's permit issued pursuant to this chapter if the licensee or permit holder:

1. (1) Is a veteran as defined in § 33A-1-1;
2. (2) Has requested the designation on the license or permit; and

3. (3) Has provided proof of the veteran's military service and honorable discharge by submitting to the department:

   1. (a) A certificate signed by a county veterans service officer on a form prescribed by the Department of Veterans Affairs;
   2. (b) National Guard Form NGB-22;
   3. (c) A Uniformed Services Identification card;
   4. (d) United States military Form DD-2;
   5. (e) United States military Form DD-2A; or
   6. (f) United States military Form DD-214.
