Approval of plans and specifications from the secretary is required for the following activities which may result in a discharge:

1. (1) The construction, installation, modification, or operation of a disposal system or part of a disposal system;
2. (2) The increase in volume or strength of wastes in excess of the limit specified in an existing SWD permit; or
3. (3) The construction or use of a new outlet for the discharge of waters into waters of the state.
