Release reports shall provide the following information:
- (1) The specific location of the release;
- (2) The type and amount of product released;
- (3) The responsible person's name, address, and telephone number;
- (4) An explanation of any response action that was taken;
- (5) The list of agencies notified;
- (6) The suspected cause of the release;
- (7) The date and time of the release to the extent known; and
- (8) The immediate known impacts of the release.
