The successor operator shall file a notice of transfer with the department. The notice shall include the following:

1. (1) A completed permit transfer application form provided by the department pursuant to § 74:29:04:02;
2. (2) The fee of $100 required by SDCL 45-6B-47; and
3. (3) A replacement surety.
