A contract for system operation pursuant to § 74:21:02:67 must contain the following provisions which shall be considered by the board in granting or denying approval:
- (1) The parties involved, including names, addresses, and phone numbers of each;
- (2) The specific starting and expiration dates of the contract;
- (3) The minimum number of visits each month and the minimum time of each visit to be spent on-site by the contract operator;
- (4) The duties and responsibilities of each party involved; and
- (5) The signature of each party to the contract.
