Persons, as specified in SDCL 43-17-28, submitting a request to determine an ordinary high or low water mark shall provide the following information:

1. (1) The authority by which the request is presented;
2. (2) The name of the person, agency, or group submitting the request;
3. (3) The name and location of the lake;
4. (4) The requested action and reasons for the action; and
5. (5) The signature of the person or the authorization of the group or agency making the request.
