Each application for a permit shall include the following information.

1. (1) The name and mailing address of the owner of the device, and the location of the interstate rest area or information center for which the permit is requested;
2. (2) The name, address, and daytime telephone number of the person responsible on a daily basis for the operation and maintenance of the device;
3. (3) The name of the publication being vended; and

4. (4) Whether the publication is daily, weekly, monthly or other publishing cycle.

   No application may be considered for approval until the application meets the requirements of this section.
