A permit-issuing authority may issue an annual permit which allows the movement on state trunk highways of a straight truck equipped with a booster axle whose loaded weight exceeds that allowed by SDCL 32-22-21 but does not exceed 600 pounds per inch of tire width. The permit may be issued under the following conditions:

1. (1) The booster axle was installed on the truck before July 1, 1996; and

2. (2) An application form is filled out which documents the following:

   1. (a) The name, address, and telephone number of the company or individual operating the truck;
   2. (b) The make, model, serial number, and model year of the truck;
   3. (c) The make, model, serial number, and year of manufacture of the booster axle;
   4. (d) The date the booster axle was installed on the truck;
   5. (e) The number of axles on the truck, including the booster axle and all center-to-center axles spacing distances;
   6. (f) The number of tires and the tire size for each axle on the truck, including the booster axle;
   7. (g) The applicant's name and signature; and

   8. (h) The date of the application.

      The permit is assigned to the truck equipped with the booster axle.
