67 : 61 : 04 : 08 . Description of treatment services. The agency shall provide , to all staff members, clients, the department, and upon request, to the public, a written description of each service offered . The description must include:

1. (1) The eligibility criteria contained in § 67:61:12:01, 67:61:13:01, 67:61:14:01, 67:61:15:01, 67:61:16:01, 67:61:17:01, or 67:61:18:01;
2. (2) The continued services criteria contained in § 67:61:07:07;
3. (3) The discharge criteria contained in § 67:61:07:09;
4. (4) The policy and procedures governing client use of alcohol or other drugs while participating in treatment; and
5. (5) The frequency and duration of services and activities offered.
