Good cause criteria for the payment of mileage to enable a claimant to obtain services from a provider located out of South Dakota consists of the following:

1. (1) The claimant's residence is located close to a bordering state;
2. (2) The claimant has moved to another state;
3. (3) Comparable services are not available in South Dakota; or
4. (4) The claimant's physician or qualified mental health professional has referred the claimant to the out-of-state provider.
