An individual is eligible for TBI services if the individual meets the following requirements:

1. (1) The individual has medical documentation from a licensed physician which contains a diagnosis of TBI, information as to the client's current physical condition, and a statement that the individual is medically stable and does not require ongoing nursing services in order to participate in the program and that the disability is likely to continue indefinitely;
2. (2) The individual requires TBI services to develop the ability to live independently within the community;
3. (3) The Department of Social Services has confirmed that the individual is eligible for and receiving Medicaid;
4. (4) The TBI service provider has completed a preplacement assessment;
5. (5) The Department of Human Services has determined that TBI services are the most appropriate services needed to address the individual's needs; and
6. (6) The individual is not receiving HCBS.
