The team shall review a child's case at least annually to determine if the child continues to meet the eligibility requirements of this chapter.

If the team determines that the child no longer meets the eligibility requirements, the case shall be closed and the child may no longer receive services under this chapter.

Failure to provide the department with required documentation concerning medical or financial information is cause for terminating eligibility.
