Long-term care assistance shall be provided to an eligible aged, blind, or disabled applicant or recipient meeting one of the following:

1. (1) The individual is at least 65 years of age at the time of application;
2. (2) The individual has a central visual acuity of 20/200 or less in the better eye with the use of a correcting lens;
3. (3) The individual has limited vision: the widest diameter of the visual field subtends an angle no greater than 20 degrees;
4. (4) The individual is unable to engage in substantial gainful activity because of a physical or mental impairment which can be expected to result in death or which has lasted or can be expected to last for a continuous period of at least 12 months; or
5. (5) The individual is a child under 18 years of age who is unable to engage in substantial gainful activity because of a mental or physical impairment which can be expected to result in death or which has lasted or can be expected to last for a continuous period of at least 12 months.
