The department and the individual shall develop an individual care plan for each individual eligible for services based on the individual's assessment. The plan must contain:

1. (1) The services or supports authorized;
2. (2) The number of service hours authorized;
3. (3) The tasks assigned to the provider; and
4. (4) The amount of any copay, if applicable.
