An individual is exempt from the preadmission screening and resident review process following a hospital discharge if:
(3) The individual's attending physician has certified, before admission to the medicaid-certified nursing facility, that the individual is likely to require less than thirty calendar days of medicaid-certified nursing facility services.
The medicaid-certified nursing facility must have a completed exempt hospital discharge form on file for an individual if the exemption criteria for any subdivision in this rule applies. If an individual enters a medicaid-certified nursing facility as an exempt hospital discharge and is later found to require more than thirty days of nursing care, the facility must request a preadmission screening and resident review prior to the expiration of that thirty days.
