An individual’s care becomes a noncovered service when the certification team determines that one of the following has occurred:

1. (2) The facility failed to provide the documentation required in § 67:16:47:08; or
2. (3) The individual is no longer eligible.

(1) The individual has reached maximum potential in the current setting;
