The state review team shall consist of at least one representative from each of the following state agencies:

1. (1) The Department of Corrections;
2. (2) The Department of Social Services; and

3. (3) The Department of Human Services.

   The team may also include a representative from the Department of Education if appropriate for the individual in care.
