As required by the American Diabetes Association and the Department of Health, the provider must prepare an initial assessment of the individual's needs, develop an education plan based on the assessment, prepare reassessments as necessary to adjust to the individual's changing needs, and conduct and document a post-program evaluation. The provider shall maintain a copy of the documents, and a copy of any order for diabetes self-management training issued by a physician or any other licensed practitioner. The provider shall make the documents and orders available to the department on request.
