Payment for nutritional items and supplies is the lesser of the provider's usual and customary charge or the applicable fee established on the department's fee schedule website. If no fee is specified for nutritional formulas, payment is limited to 60 percent of the provider's usual and customary charge. Supplies and administration kits are paid at 90 percent of the provider's usual and customary charge.

The nutritional items and supplies and their associated rates of payment are subject to review and amendment under the provisions of § 67:16:01:28.
