The department shall develop a case service plan for each individual eligible for personal care services. The plan shall be based on the individual's needs assessment and shall contain items such as the following:

1. (1) The reason for the service request;
2. (2) The number of personal care service hours assigned to the individual;
3. (3) The personal care services needed; and
4. (4) The individual's responsibilities.
