After an individual has been determined eligible for services under this chapter, the department must assess the individual's personal care service needs. The department shall complete the needs assessment at least once every twelve months. The needs assessment is based on information provided by the individual in the following areas:

1. (1) Social resources;
2. (2) Physical environment;
3. (3) Physical health; and
4. (4) Activities of daily living.
