The P and T committee's recommendations to the department for those drugs requiring prior authorization from the department must include the following information:

1. (1) A list of the recommended drugs that the department may consider to be appropriate for placement on or removal from the list of drugs that require prior approval by the department;
2. (2) For each drug listed, a preferred list of covered prescription drugs within the appropriate therapeutic classes for a particular disease or condition; and

3. (3) The prior authorization criteria.

   When developing its preferred list of covered prescription drugs, the committee must consider the clinical efficacy, safety, and cost-effectiveness of the product.
