A claim for services covered under § 67:16:12:02 must be submitted according to the following:

1. (1) If the claim relates to a physician's service, follow the claim requirements of chapter 67:16:02;
2. (2) If the claim relates to hospital services, follow the claim requirements of chapter 67:16:03;
3. (3) If the claim relates to home health services, follow the claim requirements of chapter 67:16:05;
4. (4) If the claim relates to clinic services, follow the claim requirements of chapter 67:16:13;
5. (5) If the claim relates to prescription drugs, follow the claim requirements of chapter 67:16:14;
6. (6) If the claim relates to ambulatory surgical center services, follow the claim requirements of chapter 67:16:28; and
7. (7) If the claim relates to federally qualified health centers and rural health clinic services, follow the claim requirements of chapter 67:16:44.
