A claim submitted under this chapter must be submitted at the provider's usual and customary charge and in accordance with the applicable provisions of chapters 67:16:01, 67:16:02, 67:16:03, 67:16:05, 67:16:13, 67:16:14, 67:16:28, and 67:16:44.
