Good cause criteria for voluntarily leaving employment or reducing earned income is established if:
- (1) Continued employment or full-time work is a hazard to the applicant's or recipient's health;
- (2) The employer required the applicant or recipient to relocate the applicant's or recipient's residence to hold the job;
- (3) The employer's conduct demonstrated a substantial disregard of the standards of behavior that the applicant or recipient as an employee has a right to expect from the employer;
- (4) The employer breached or substantially altered the employment contract; or
- (5) While on layoff, the applicant or recipient accepted employment that the applicant or recipient quit in order to return to work for the applicant's or recipient's regular employer.
