The department shall furnish each assistance unit with a monthly report form. A member of the assistance unit must fill out, sign, and return the monthly report form to the department. A form returned by mail must be postmarked by the twentieth of the month before the month of payment. The monthly report form must contain the information necessary to allow the caseworker to determine eligibility and compute the amount of assistance payment.
