The department shall note the applicant's or recipient's case record of supportive facts for each decision regarding eligibility or ineligibility except when:

1. (1) The applicant voluntarily withdraws the application, and there is an entry in the case record that a notice has been sent to confirm the applicant's notification to the agency that the applicant has withdrawn the application; or
2. (2) There is an entry in the case record that the applicant died or could not be located and the application is deemed withdrawn.
