Application for duplicate certificate of title may be made by the owner of record or by an authorized agent holding power of attorney. The agent shall sign the application with the name of the owner of record and after the signature insert the words "power of attorney" or another court-appointed title. A copy of the power of attorney document or other documents required by the department shall be attached to the application form.
