A person or entity required to report hazardous substances and remit the fee imposed by SDCL 34-12-21 shall file the report on the forms which include the following:
- (1) The name and address of the person or entity making the report;
- (2) The signature of a principal of the filer;
- (3) The status of the filer under SARA Title III;
- (4) The number of chemicals or mixtures and, in the case of filers reporting release information pursuant to section 313 of SARA Title III, quantity of hazardous substances handled or released;
- (5) The computation of the fee remitted;
- (6) The period covered by the report; and
- (7) Any other information that permits the department to determine whether the fee is properly computed and reported.
