ARSD 55:05:05:04 – Affirmation of policy form.

Each employee shall read, sign, and date an affirmation of policy form at the time the employee is informed of the provisions of this article and before any drug screen test is conducted on the employee. The affirmation of policy form describes the drug screening program.
