Each recipient is responsible for proper care of telecommunications equipment provided by the program, and an authorized provider may determine a recipient ineligible for continued or future participation if:

1. (1) Equipment has been subjected to abuse, misuse, or unauthorized repair by the recipient;
2. (2) Each recipient failed to provide a police report of stolen equipment; or

3. (3) The recipient has lost previously issued equipment.

   Each recipient must follow the manufacturer's recommended maintenance procedures for all equipment and must report any problems with the equipment to the provider.
